If you are interested in sending your child or children to our school, please visit us at your soonest convenience. To setup an an appointment, please go to our Contact page for our phone number.

In order to be considered for enrollment:

  1. The application form must be filled out.  Click on one of the following links to print out a form:
  2. Admission test must be taken
  3. All required documents must be submitted to the office
  4. The family agreement must be signed

The following documents must be submitted by email, fax, postal mail, or drop-off.

  • Completed Application
  • Birth Certificate
  • Baptismal Certificate
  • Immunization Records
  • Physical Exam Certificate
  • Sacramental Certificates
  • Previous School’s Records
  • Previous School’s Report Card
  • Previous School’s Conduct Reports and Referral Letters
  • Complete FACTS contract
  • Name of referring OLV family, if applicable

Registration Fee

A registration fee of $250 will be due for each student.  The registration fee is nonrefundable.
